The graduated approach, made practical

Assess Plan Do Review follows the cycle schools already use, and turns the paperwork into a few clicks.

Assess

Pick the barriers you have seen, or paste notes from a report.

Plan

Choose quality first teaching strategies and agree outcomes with the pupil.

Do

Share a one-page profile so every teacher knows what works.

Review

Use review questions to check progress and set the next steps.

  1. Add the barriers. Paste notes from a report or pick from 50 barriers across the four areas of need.
  2. Choose strategies. Review quality first teaching strategies for each barrier, for the pupil's primary and secondary area of need, and for the pupil's phase, from Reception to sixth form.
  3. Agree targets. Edit ready-made Assess, Plan, Do, Review target templates, then copy the plan or download it as a PDF.
  4. Share a one-page profile. Give every teacher a short summary with passes, healthcare and evacuation plans, access arrangements for tests and exams, reading age and other support in place.

What is in it

Everything a SENCo, pastoral lead or teaching assistant needs to write a clear, useful plan.

Import an EHCP

Add a pupil's EHCP as a PDF, Word or text file. Needs, outcomes and provision are matched to a draft plan for you to check. The file never leaves your device.

Evidence-informed

Strategies are tagged against the EEF's five-a-day for high quality teaching, targets are checked for being measurable and time-bound, and plans follow the SEND Code of Practice graduated approach.

Shaped by your school

Your vision, values, priorities, provision menu and the name you use for plans run through every plan your staff write.

Barriers and strategies

50 barriers to learning across all four areas of need, with quality first teaching strategies for each phase, assess prompts and target templates.

Pupil profile

A one-page profile with the pupil's voice, strengths, agreed passes, optional photo and the plans in place.

Early years log

An Assess, Plan, Do, Review log built around the EYFS areas, for nurseries, pre-schools, childminders and Reception.

Primary and secondary

Barriers and strategies that match the pupil's phase, from phonics and early reading to revision and exam preparation.

Transitions

Strategies for starting school, moving from Year 6 to secondary and settling into Year 7.

Medical and physical needs

Prompts for healthcare plans, personal care plans, emergency evacuation plans and risk assessments.

Access arrangements

Record adjustments for KS2 tests, GCSEs and other exams, and the evidence behind them.

Your school's name and crest

Each school gets its own address, name, crest and colours, with its own staff and booklet links.

Teaching assistant guide

Best practice, what to expect from an Ofsted visit, and example questions and answers.

Progress dashboard and insights

See planned against achieved targets for every cycle, which areas of need progress fastest and which targets keep carrying forward. Worked out on the device: no AI service sees pupil data.

Works with your tools

Add reviews to Google Calendar or Outlook, set Slack or WhatsApp reminders, and export actions to Asana, Jira, Notion or Trello.

Pupil premium and free school meals

Record pupil premium, early years pupil premium and free school meals status, and how the funding supports each pupil. Staff can leave it off the printed plan.

Other support in place

Record outside agencies and in-school support such as speech and language therapy, homework club and pastoral groups.

Privacy

Assess Plan Do Review stores no pupil information and sends no pupil information anywhere. What staff type in is held only in their browser while the page is open and disappears when it is closed. There are no pupil accounts and no database. The planner itself sets no cookies and uses no analytics or AI services. School addresses use a sign-in that sets one cookie to remember you have signed in. Only what a member of staff chooses to copy or download leaves the page, and your school stays in charge of those files.

Read the full privacy statement and the answers your data protection officer will ask for.

Assess Plan Do Review is a planning aid. It gives starting points for professional judgement and is not a diagnosis or a substitute for specialist assessment.

Questions schools ask

Does it store anything about our pupils?

No. What staff type is held only in the browser while the page is open. Staff copy or download the finished plan and keep it in the school's own systems.

Does it work for early years and primary?

Yes. Choose the pupil's year, from Reception to Year 13, and you see barriers and strategies for that phase. Nurseries, pre-schools and childminders can use the early years log, built around the EYFS areas.

Can it carry our school's name and crest?

Yes. Each school gets its own web address, with its name, crest and colours and links to its own booklet and staff contacts.

Does it replace professional judgement?

No. It is a planning aid that offers starting points. A SENCo or teacher decides what is right for each pupil, and specialist assessment still has its place.

Who can open our school's version?

School addresses sit behind a staff sign-in using a one-time code sent to a school email address.

Can we try it first?

Yes. Use the demo with a made-up pupil, or ask us about a pilot for your school.
